The FAIRMONT is located in West Midtown, at 1429 Fairmont Avenue, Atlanta GA 30318.

This special event will be held in celebration of the Center being recently recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance. The Patient-Centered Medical Home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams. In addition to celebrating this great accomplishment, we will be raising funds to improve care and upgrade equipment for our medical and dental areas.

Our evening will begin at 6:00 PM with a wine and beer reception featuring the most delicious hors d’oeuvres prepared by Bold Catering and the engaging sounds of the Atlanta based string quartet, Cobalt Strings.

Dinner with a uniquely designed menu by Bold (voted Best Atlanta Caterer by the AJC and Creative Loafing) and program will begin at 7:00 PM.
